•$140,000 - $180,000

About The Position

Circadia Health is a growth-stage healthcare AI company focused on preventing avoidable hospitalizations and transforming senior-care operations. Its Circadia Intelligence Platform combines contactless respiration and motion sensing, predictive models, and enterprise integrations with clinical and administrative workflows. The technology serves more than 40,000 post-acute patients daily across skilled-nursing, home-health, and home-care networks. The Nurse Practitioner will provide on-site care in Middletown, Connecticut, using predictive health signals to support earlier interventions and reduce preventable readmissions. The role combines direct patient care with integration of technology into bedside workflows, including staff training, live demonstrations, and shadowed assessments. The NP will round across designated skilled nursing facilities, typically completing 30 or more visits per day where appropriate, and may cover sites within a 30–40-mile radius. Mileage and lodging are covered per policy for sites outside that radius. Success is measured by patient visits, engagement, time to acknowledgement, timely documentation, facility workflow adoption, and trends in falls and readmissions. The position is full-time, employed by Circadia Health, Inc., with an anticipated annual base salary of $140,000–$180,000. Compensation placement depends on job-related factors including skills, qualifications, experience, and location.
