About The Position

St. Catherine of Siena Hospital is currently looking for an experienced APP/MLP Nurse Practitioner to join our team of primary care specialists committed to providing advanced, high quality healthcare in an outpatient setting. Requirements

  • Graduate of an accredited school of nursing.
  • Graduate of an accredited program for Certified Nurse Practitioner.
  • Current licensure by N.Y.S. Dept. of Education to practice as RN and NP, Master's Degree and Certification required.
  • BLS required.

  • Bachelors Degree preferred.
  • Family Nurse Practitioner preferred.

Responsibilities

  • Coordinates the initiation of a patient-focused team in providing direct and collaborative health care.
  • Performs physical examination and comprehensive patient assessment on pre-surgical patients.
  • Collects and collages all pertinent patient information related to diagnosis and demographics.
  • Initiates patient on the designated clinical pathway.
  • Ensures that all tests, therapies and treatments associated with the pathway are begun.
  • Demonstrates care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights.
  • Performing patient assessments and providing appropriate ordering of testing and medications, along with formulating an appropriate plan of care.
  • Admitting patients.
  • Performing H&P's.
  • Coordinating transfers to and from other facilities.
  • Collaborates with interdisciplinary team regarding discharge planning and plan of care.
  • Navigate oneself throughout the institute, be able to stand/ walk for extended periods of time.
  • Record and report information.
