Nurse Practitioner

Circadia Health•New Jersey, NJ

About The Position

Circadia Health is a growth-stage healthcare AI company whose platform monitors patients and predicts adverse events to help prevent avoidable hospitalizations and improve senior-care operations. Its technology serves post-acute care patients across skilled-nursing, home-health, and home-care networks. The Nurse Practitioner provides on-site care at partner facilities in Central New Jersey, combining direct patient care with the integration of predictive technology into bedside workflows. The role includes assessing residents flagged by clinical signals, acting on trend-based insights, coaching facility staff, and supporting timely interventions intended to reduce preventable decline and readmissions. This is a full-time, in-field position requiring presence at partner facilities Monday through Friday. The NP completes high-volume rounds across designated skilled-nursing facilities, generally targeting 30 patient visits per day where appropriate. Sites may be within a 30–40-mile radius; mileage and lodging for sites outside that radius are covered per policy. Success is measured by visit volume, facility engagement, acknowledgement response time, timely documentation, workflow adoption, and trends in falls and readmissions.
