About The Position

Community Health Network is seeking an experienced, family medicine-certified advanced practice provider for an Employer Health Onsite primary care role serving onsite employees and their beneficiaries on the south side of Indianapolis. The position floats among South Side Onsite Care Clinics as needed, with a home base at the University of Indianapolis, 1643 E Hanna Avenue, Indianapolis, IN 46060. The role involves providing preventive and routine primary care, diagnosing and treating illness or injury, managing chronic or complex conditions, medication management, and referring patients to specialists when appropriate. The position uses the EPIC electronic medical record and does not require holiday work.

Requirements

• Family medicine certification.
• At least two years of experience as an advanced practice provider.
• Educational background consistent with requirements for licensure and certification in the State of Indiana.
