Our client is a growing healthcare organization focused on providing high-quality physiatry and pain management services within the skilled nursing facility setting. Their clinical team works closely with rehabilitation professionals to help patients improve mobility, recover from complex medical conditions, and regain independence. Our client is looking for a motivated Nurse Practitioner to provide physiatry and pain management services within skilled nursing facilities in the Richmond, Virginia area.
Requirements
- Active NP License in Virginia

- Experience in PM&R/Physiatry, Pain Management, Geriatrics, or Post-Acute Care
Responsibilities
- Provide On-Site Physiatry and Pain Management Services at Skilled Nursing Facilities
- Evaluate Patients and Develop Individualized Care Plans
- Manage Patients With Pain, Spasticity, and Functional Limitations
- Develop Treatment Plans Focused on Mobility and Recovery
- Treat Patients Recovering From Stroke, Surgery, and Complex Medical Conditions
- Consult With Rehabilitation Teams and Collaborate With PT/OT/ST
- Coordinate Care With Interdisciplinary Teams
- Focus on Improving Functional Outcomes and Reducing Rehospitalizations
- Provide Patient-Centered Care Focused on Functional Recovery and Independence
- Work a Predictable Weekday Schedule With No Required Evenings or Weekends
