Their clinical team works closely with rehabilitation professionals to help patients improve mobility, recover from complex medical conditions, and regain independence. Our client is looking for a motivated Nurse Practitioner to provide physiatry and pain management services within skilled nursing facilities in the Lynchburg, Virginia area.
Requirements
- Active NP License in Virginia

Responsibilities
- Provide On-Site Physiatry and Pain Management Services at Skilled Nursing Facilities
- Evaluate Patients and Develop Individualized Care Plans
- Manage Patients With Pain, Spasticity, and Functional Limitations
- Develop Treatment Plans Focused on Mobility and Recovery
- Treat Patients Recovering From Stroke, Surgery, and Complex Medical Conditions
- Consult With Rehabilitation Teams and Collaborate With PT/OT/ST
- Coordinate Care With Interdisciplinary Teams
- Focus on Improving Functional Outcomes and Reducing Rehospitalizations
- Provide Patient-Centered Care Focused on Functional Recovery and Independence
- Work a Predictable Weekday Schedule With No Required Evenings or Weekends
