About The Position

We are seeking Nurse Practitioners (Certain NPs only; no PAs) with no less than 40 hours per month who can start immediately completing in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Must be able to complete credentialing paperwork, training, and onboarding within the first 10-12 days and start field-based work immediately after thereafter. This is a pure assessment role: No prescribing, no on-call, and zero follow-up care.

Requirements

• Active NP license (No PAs; most specialties accepted except Pediatrics, PMHNP, and Women's Health).
• Reliable personal transportation to travel locally across multiple counties.
• Ready to start ASAP.
• Must be able to complete credentialing paperwork, training, and onboarding within the first 10-12 days.
• Must be willing to cover 2.5 counties.

Responsibilities

• Conduct in-home HRAs (medical history, lifestyle, basic physical/mental evaluations).
• Provide preventive care education.
• Document visits using a company-provided iPad.
