About The Position

Extendicare is seeking independent, collaborative, and clinically skilled Nurse Practitioners (NPs) who demonstrate advanced critical thinking, strong communication, and a commitment to excellence in geriatric care. The NP functions as an independent practitioner accountable to residents, College of Nurses of Ontario (CNO) standards, and Extendicare’s policies. The NP works closely with the interdisciplinary team to assess, diagnose, treat, and evaluate clinical care for residents with acute and chronic health needs. The NP practices within the extended class scope, identifying personal care parameters and consulting or referring to physicians and other health professionals as appropriate.

Requirements

  • Nurse Practitioner (RN Extended Class) with current and unrestricted registration with the College of Nurses of Ontario (CNO).
  • Master's Degree with post-Master’s NP certificate in NP-Adult or NP-Primary Health Care.
  • Professional liability insurance.

Nice To Haves

  • Membership in NPAO preferred.
  • Experience in primary care, gerontology, dementia care, and/or complex care an asset.

Responsibilities

  • Conduct comprehensive assessments, including health histories, mental status evaluations, and physical examinations.
  • Integrate resident, family, and staff perspectives with nursing and allied health approaches and evidence-based practice.
  • Formulate and collaborate on acute and chronic diagnoses; consult with physicians when appropriate.
  • Order and interpret screening and diagnostic tests and make clinical decisions based on results.
  • Develop, implement, and evaluate individualized care plans in collaboration with interdisciplinary team members.
  • Prescribe pharmacological and non‑pharmacological therapies.
  • Foster therapeutic relationships that promote independence and well‑being.
  • Adjust care plans based on resident outcomes.
  • Collaborate with interdisciplinary teams, leadership, and community partners.
  • Provide consultation and guidance to staff on complex clinical cases.
  • Refer to dietitians, physiotherapists, pharmacists, and other interdisciplinary team members as needed.
  • Participate in advisory committees and quality initiatives.
  • Educate residents and families to support autonomy and informed decision‑making.
  • Support staff education through identification of care trends.
  • Mentor and guide team members and learners.
  • Act as an innovator, advocate, and change agent.
  • Contribute to the home’s mission, vision, values, and goals.
  • Analyze resident care trends and advocate for system improvements.
  • Participate in quality improvement, research, and practice evaluation.
  • Ensure compliance with CNO standards and best practice guidelines.
  • Maintain accountability for professional practice.
