What it's like to be a Nurse Practitioner/Physician Assistant at Nurse Practitioner/Physician Assistant Step into a rewarding role as a Medical Provider on our Atlas Medical Care Team, where your mission is to deliver mobile on-site primary care to vulnerable patients in assisted living and senior communities. In this dynamic position, you'll autonomously manage complex healthcare scenarios, addressing the challenges of multiple co-morbid conditions with confidence. Beyond medical care, you’ll play a crucial role in educating patients and their families on self-care, disease management, and end-of-life discussions. Collaboration is at the heart of our team approach, as you will work closely with facility staff and our Atlas Patient Care support teams. Requirements

Minimum of one year of primary care experience.
Valid licensure as a Nurse Practitioner or Physician Assistant in the state of practice.
Ability and desire to work independently and drive between communities.
Proficiency in using an EMR system (Elation is currently used).
Strong verbal and written communication skills.
Excellent time management abilities.
Ability to pass a Criminal Background Check.
Current CPR certification.
Proof of a negative TB test.

Responsibilities

Deliver mobile on-site primary care to vulnerable patients in assisted living and senior communities.
Autonomously manage complex healthcare scenarios, addressing the challenges of multiple co-morbid conditions.
Educate patients and their families on self-care, disease management, and end-of-life discussions.
Collaborate with facility staff and Atlas Patient Care support teams.
Uphold confidentiality and maintain high standards of care.
