About The Position

Humana's Primary Care Organization provides senior-focused, value-based care through the CenterWell and Conviva brands. As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will work onsite at the Jourdanton clinic as part of a collaborative, multidisciplinary team. You will provide comprehensive care to seniors in accordance with clinical standards and contribute to coordinated, personalized, high-quality care. The position reports to the Associate Medical Director.

Requirements

• Master's degree in Nursing or completion of a Physician Assistant program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Must be screened for TB if selected for the role.

Nice To Haves

• Two or more years of experience in value-based care, primary care, or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain a DEA license and/or Medicaid and Medicare numbers.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital and skilled nursing facility coordination, durable medical equipment, and home health services.
• Document care accurately and efficiently, with support from quality-based coders.
• Participate in the on-call rotation.
• Support growth plans at the center.
• Provide value-based care.
