VNA is looking for a Nurse Practitioner. Job Summary: The Nurse Practitioner (NP) will provide clinical care, education, consultation, and support for research initiatives, while working collaboratively with physicians and other members of the multidisciplinary team. Responsibilities include coordinating patient care, overseeing individualized plans of care, conducting face-to-face home visits, completing clinician documentation, and providing follow-up care. The NP will serve as a co-primary clinician, in collaboration with VNA Medical Directors, for patients who do not have an established primary clinician. The role also includes conducting physician/NP-level home visits for patients whose primary remains responsible for their care and providing telephone and -on-site clinical consultation to staff as assigned.

Requirements

  • Current California Nurse Practitioner license
  • California Furnishing license and DEA certificate
  • Master’s degree in nursing or equivalent advanced practice certification
  • CPR certification
  • Strong knowledge of pain and symptom management principles
  • Excellent interpersonal, communication, and clinical consultation skills
  • Current California driver's license
  • Auto insurance
  • Reliable vehicle
