About The Position

Crosslands is seeking a Certified Registered Nurse Practitioner to provide medical care and services to residents in Independent Living in a medical office setting. The role works with Kendal~Crosslands Communities primary physicians under a Collaborative Practice Agreement. Staff have the opportunity to build caring, meaningful relationships with residents in a Quaker value-centered, family-atmosphere work environment. This is a full-time position, five days a week, with an occasional four-hour Saturday shift and no holidays.

Responsibilities

Provide on-site medical care and services to community residents in collaboration with primary physicians under a Collaborative Practice Agreement.
Identify and manage chronic and acute health conditions by requesting laboratory and/or x-ray studies, initiating treatment, managing medication regimens, and arranging appropriate follow-up.
Respond to emergencies, administer first aid, evaluate injuries, and arrange further treatment when necessary.
Participate in planning for residents’ needs as they transition through levels of care.
Collaborate with the Director of Resident Care regarding present or anticipated problems.
Perform medical procedures as necessary.
Follow pharmacy protocols and procedures, including e-prescribing.
Administer prescribed or necessary treatments, injections, and medications to residents.
Communicate laboratory and test results to residents and physicians.
Manage INR laboratory results and arrange follow-up as necessary.
Provide wellness screenings.
Initiate requests for consultations or referrals.
Participate in Community Quality Assurance, Pharmacy, and Infection Control Committees as requested by the Director.
Arrange and establish orders for transfers to the Health Center or hospital, and assist with discharges from hospital back to Independent Living and/or Skilled Nursing.
