About The Position

LifeSpan Advanced Care is seeking a Nurse Practitioner to provide comprehensive healthcare services to a diverse resident population at Greenwood Health and Living. The role involves assessing resident health, diagnosing conditions, developing individualized treatment plans, and collaborating with interdisciplinary teams to manage acute and chronic illnesses. The Nurse Practitioner also educates and supports residents in managing their health, with the goal of improving quality of life while maintaining high standards of clinical practice and resident safety.

Requirements

• Master's degree in nursing (MSN) or higher from an accredited program.
• Current Nurse Practitioner (NP) licensure and certification in the state of Indiana.
• Basic Life Support (BLS) certification.
• Demonstrated experience in resident care, including vital signs monitoring and wound care.
• Proficiency in basic computer literacy, including electronic health record (EHR) systems.

Nice To Haves

• Certification in Infection Control or a related specialty.
• One year of experience working in long-term care or geriatric healthcare settings.
• Advanced certifications such as Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
• Familiarity with quality improvement initiatives and healthcare compliance standards.
• Strong communication and leadership skills to support interdisciplinary team collaboration.

Responsibilities

• Conduct thorough resident assessments, including taking vital signs and reviewing medical histories.
• Diagnose and treat acute and chronic illnesses, injuries, and other health conditions.
• Develop and implement individualized care plans, including wound care and infection control protocols.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate resident care.
• Document resident information accurately using electronic health records and maintain compliance with healthcare regulations.
• Provide resident education on disease prevention, medication management, and healthy lifestyle choices.
• Monitor resident progress and adjust treatment plans as necessary to ensure effective outcomes.
