About The Position
This is a daytime Nurse Practitioner role focused on providing care in skilled nursing and long-term care facilities. The position involves managing chronic conditions, performing full assessments, closing care gaps (HEDIS and STAR measures), and leading care planning with facility teams. The role also includes supporting and educating patients' families. The Nurse Practitioner will work onsite at partner facilities in Saginaw, MI, and will have autonomy over their patient panel and pace.
Requirements
- Active, unrestricted APN license in Michigan
- Board certified and Medicare credentialed
- 6+ months in skilled nursing or long-term care
- Solid with digital tools and team-based care
- Able to work onsite in Saginaw, MI
Responsibilities
