About The Position

Circadia Health is seeking a full-time Nurse Practitioner to conduct remote assessments for home health patients. This role involves acting on predictive health signals in real-time and coaching care teams through virtual touchpoints to reduce hospitalizations. The position is hybrid, requiring three days a week seeing patients in their homes and two days a week delivering care via telehealth. The Nurse Practitioner will interpret insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions. Responsibilities include completing documentation within the same day, managing 25+ tele visits per day for home health patients in the SoCal/OC area, and providing feedback to improve virtual workflows and tech integration. Success will be measured by visit targets, timely addressing of flagged patients, documentation completion, and workflow adherence. The role also involves mentoring new NPs and collaborating with clinical operations and product teams.

Requirements

• Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.
• Comfortable conducting telehealth visits and navigating virtual care platforms.
• Thrives in a dynamic, evolving startup environment.
• English required.

Nice To Haves

• Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools.
• Spanish preferred.

Responsibilities

• Conduct initial and follow-up telehealth assessments on flagged patients.
• Initiate/adjust plans of care.
• Coordinate with referring and primary care providers via secure video/audio platform.
• Interpret trend-based insights from the Circadia platform.
• Prioritize high-risk patients and drive timely interventions to prevent decline and unnecessary ED visits.
• Complete documentation/charts within the same day.
• Efficiently complete 25+ tele visits per day for designated home health patients in the SoCal/OC area, focusing on new admissions, high-acuity flagged patients, and Circadia alerts.
• Surface insights to improve virtual workflows and tech integration in a home health setting.
• Support NP onboarding, share best practices, and partner with clinical ops/product teams.
