Nurse Practitioner - Full-time (Home Health)
About The Position
Circadia Health is seeking a full-time Nurse Practitioner to conduct remote assessments for home health patients. This role involves acting on predictive health signals in real-time and coaching care teams through virtual touchpoints to drive earlier interventions and reduce preventable hospitalizations. The position is hybrid, requiring three days a week seeing patients in their homes and two days a week delivering care via telehealth. The Nurse Practitioner will be responsible for remote patient care, utilizing data insights from the Circadia platform to prioritize high-risk patients, completing documentation, managing tele-visits, providing quality and product feedback, and mentoring/collaborating with other team members.
Requirements
- Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation; company bills on your behalf.
- 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.
- Comfortable conducting telehealth visits and navigating virtual care platforms.
- Thrives in a dynamic, evolving startup environment.
- English required.
Nice To Haves
- Tech-savvy: Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools.
- Spanish preferred.
Responsibilities
- Conduct initial and follow-up telehealth assessments on flagged patients via secure video/audio platform.
- Initiate/adjust plans of care.
- Coordinate with referring and primary care providers.
- Interpret trend-based insights from the Circadia platform and prioritize high-risk patients.
- Drive timely interventions to prevent decline and unnecessary ED visits.
- Complete documentation/charts within the same day.
- Efficiently complete 25+ tele visits per day for designated home health patients, focusing on new admissions, high-acuity flagged patients, and Circadia alerts.
- Surface insights to improve virtual workflows and tech integration in a home health setting.
- Support NP onboarding, share best practices, and partner with clinical ops/product teams.
