In-Home Nurse Practitioner (Part Time) - Colby, KS
About The Position
Signify Health, part of CVS Health®, is seeking a Part-time Nurse Practitioner to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. The Nurse Practitioner will use an iPad with an intuitive clinical workflow and is not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and other airborne allergens.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification.

Responsibilities
- Provide In-Home Health Evaluations.
- Connect one-on-one with patients and answer health questions.
- Conduct medication and medical history reviews.
- Perform physical evaluations.
- Conduct additional diagnostic tests if ordered by the patient's health plan (e.g., diabetic eye exams, spirometry).
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Travel locally and within licensed states, as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
