In-Home Nurse Practitioner (Part Time) - Salina, KS
About The Position
Signify Health, part of CVS Health®, is seeking a Part-time Nurse Practitioner to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order labs, or alter current treatment plans. The primary workplace is the patient's residence, which can vary in conditions, including the presence of pets, secondhand smoke, and airborne allergens.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).

Responsibilities
- Provide In-Home Health Evaluations.

- Conduct flexible, comprehensive, and personalized health evaluations.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
- Be flexible to travel locally and within licensed states, as needed.
