About The Position

The Nurse Practitioner (NP) provides high-quality, patient-centered primary care services to a diverse population, including individuals with special healthcare needs. The NP practices in accordance with state regulations, FQHC standards, and within the defined scope of practice, collaborating with physicians, behavioral health providers, and other members of the care team to optimize patient outcomes.

Requirements

  • Active State Registered Nurse (RN) license.
  • Completion of an accredited Nurse Practitioner program.
  • Board certification in applicable NP specialty.
  • Prescriptive authority and current BLS certification.

Nice To Haves

  • Master of Science in Nursing (MSN) preferred, unless grandfathered.
  • Minimum of two (2) years of primary care experience preferred.
  • Experience in an FQHC or community health setting preferred.

Responsibilities

  • Conduct comprehensive assessments including medical history, physical exams, and psychosocial evaluations.
  • Develop, implement, and document individualized care plans.
  • Order, interpret, and follow up on laboratory and diagnostic tests.
  • Prescribe medications and administer injections as authorized.
  • Make referrals to specialists and community resources as appropriate.
  • Provide patient education and counseling on health promotion, disease prevention, and treatment adherence.
  • Evaluate effectiveness of treatments and adjust care plans as needed.
  • Triage and respond to patient phone calls and medical inquiries.
  • Manage urgent and emergent medical situations within the scope of practice.
  • Collaborate closely with designated physicians, behavioral health providers, and other clinical staff.
  • Advocate for patient needs within the organization and the community.
  • Participate in team-based care planning and case discussions.
  • Function independently within the NP scope of practice.
  • Maintain flexibility to work varying schedules as required by patient and organizational needs.
  • Participate in staff meetings, clinical trainings, quality improvement activities, and continuing education initiatives.
  • Engage in data collection, interpretation, and integration for quality assurance and reporting purposes.
