About The Position
Community Health Network is seeking an experienced, family medicine certified advanced practice provider to practice PRN at their Employer Health Onsite clinics on the south side of Indianapolis, IN. This role focuses on the well-being of onsite employees and their beneficiaries within one of the nation’s most integrated healthcare systems. The position offers flexibility to float to multiple onsite clinics and schools, with no holidays required. Electronic Medical Record (EPIC) will be utilized, and competitive compensation is offered.
Requirements
- Minimum two years of experience as an advanced practice provider
- Family medicine certification
- Educational background consistent with requirements for licensure and certification with the State of Indiana.
Responsibilities
- Routine check-ups and preventive care
- Diagnosis and treatment of illness or injury
- Managing chronic or complex health conditions
- Medication management
- Referral to specialists for more serious conditions
