The Nurse Practitioner (NP) in Palliative Care facilitates clarification of patient and family goals of care and assists with access to appropriate support models of care, such as hospice, rehabilitation, and ongoing aggressive treatment. This role involves assisting with the diagnosis, treatment, and management of acute and chronic health conditions. The NP consults with the palliative care physician as needed, informs the primary physician of recommended services, and collaborates with other physicians. Additionally, the NP educates facility staff, co-workers, and the community on palliative care services, attends Palliative Medical Associates team meetings, and consults with referring physicians and acute care teams to contribute to the patient's plan of care. Responsibilities include preparing and maintaining accurate patient records, prescribing medications (including controlled substances as delegated and licensed after collaboration), and ordering diagnostics/treatments as needed after collaboration. The NP may also assist in developing clinical practice guidelines and standards for quality palliative care.
