The Nurse Practitioner assesses, treats, solves problems, counsel, and educates hospice and palliative care patients, utilizing advanced assessment and evaluation techniques. This role involves performing consultative visits for pain and symptom management, offering clinical changes to the plan of care in collaboration with the attending physician and/or medical director, and communicating these changes to the care team. The Nurse Practitioner will also confer with medical directors to review diagnosis, assessment criteria, and issues of pain and symptom management for the population served, and participate in the agency's quality improvement process. They will initiate immediate orders with the consultation of the attending medical director, assess health status and potential, and plan, implement, and evaluate health care needs of the patient/family. The role includes planning and evaluating therapeutic regimens to minimize pain and suffering, performing history and physical examinations, identifying normal and abnormal findings, and consulting with physicians on the plan of treatment. Standard precautions must be practiced in all patient contacts, and emotional support should be provided to patients and families in a manner that fosters independence and affirms existing strengths. Care provided and patient/family response must be documented in a timely fashion according to agency policy. The Nurse Practitioner will deliver care in a cost-effective manner, demonstrating an understanding of hospice reimbursement methodologies, and perform palliative care consults in hospitals, homes, and nursing facilities.
