About The Position

Bergen New Bridge Medical Center is seeking a Nurse Practitioner for Off Site Services to provide comprehensive, compassionate, and evidence-based primary care services to individuals residing in County Correctional Facility and County Shelter. Under the direction of the BNBMC Occupational Health Medical Director and County Facility Leadership, the Nurse Practitioner will exercise sound clinical judgment and advocate for the health and safety of patients while balancing the operational requirements of the correctional and shelter environments. The role involves providing comprehensive care management, chronic disease management, and preventative care and wellness using the nursing process (assessment, planning, intervention, evaluation, and teaching). The Nurse Practitioner will complete assessments, order and interpret tests, prescribe medication, provide treatments, and manage follow-up care. Responsibilities also include completing employee physicals, maintaining medical records, promoting productivity, providing patient education, adhering to policies, participating in performance improvement initiatives, and fostering teamwork. The position requires interpreting x-rays and lab values, making case presentations to physicians, developing problem lists, documenting patient interactions, providing discharge instructions, and participating in quality improvement efforts. The role also emphasizes adherence to the Medical Center’s Code of Conduct, Mission, Vision, and Values, and maintaining excellent customer service standards.

Requirements

• Graduation from an approved Nurse Practitioner program is required.
• Current NJ licensure and certification as a Nurse Practitioner.
• BLS, ACLS, and PALS certification required
• Professional knowledge of nursing theory and practice, extensive knowledge of physical assessment, differential diagnosis, pathophysiology, and pharmacology.
• Good oral and written communication skills.
• Good interpersonal communication skills.
• Speaks, reads, and writes English to the extent required by the position.

Responsibilities

• Serve as a clinical representative providing comprehensive, compassionate, and evidence-based primary care services to individuals residing in County Correctional Facility and County Shelter.
• Exercise sound clinical judgment and advocate for health and safety of patients while balancing operational requirements of the correctional and shelter environments.
• Provide comprehensive care management, chronic disease management, preventative care and wellness.
• Utilize the nursing process (assessment, planning, intervention, evaluation, and teaching) in providing care.
• Complete assessments including history and physical, medication reconciliation, problem list and develop a plan of care.
• Perform assessment and plans for ongoing treatment for patients in the service.
• Order lab and diagnostic tests for patients as appropriate to formulate diagnosis, develop treatment plans.
• Review results of lab and diagnostic tests and make adjustment to the treatment plan accordingly.
• Prescribe medication appropriate for the condition.
• Provide treatment appropriate for the condition i.e.: wound care, respiratory treatments, basic emergency treatments.
• Plan a response for follow-up for patients with abnormal tests results and intervenes with patient according to medical center policy and procedure.
• Refer to specialist, collaborate with care team for follow up care.
• Complete employee physicals as assigned.
• Assure all entries to the medical record, paper or electronic, are appropriate, timed, dated, signed and legible.
• Promote throughput for the service by achieving and maintaining medical center expectations for productivity.
• Provide patient education and feedback to promote health and wellness.
• Adhere to all medical center policies and procedures and bylaws of the medical staff.
• Participate in department performance improvement initiatives, collect and aggregate data as needed, work with leadership to develop a plan of correction.
• Foster and promote teamwork with other staff members of the service.
• Communicate pertinent information to the supervisor of the service.
• Perform comprehensive physical, psychosocial, and mental examinations, including developmental and functional status determination.
• Interpret x-rays, with assistance from physicians, in order to formulate diagnosis.
• Interpret lab values with order to correct abnormalities.
• Evaluate consultations and make concise case presentations to physicians, differentiating normal from abnormal findings and suggest differential and presumptive diagnosis.
• Develop a prioritized comprehensive problem list.
• Document in the patient’s record all pertinent patient interactions and care.
• Provide discharge instructions with prescriptions given and follow-up visits arranged.
• Participate in interdisciplinary quality improvement efforts to improve clinical practice.
• Understand and adhere to the Medical Center’s Code of Conduct.

• Maintain customer service standards: respect, flexibility, knowledge, confidence, professionalism, and pleasant attitude, patience and helpfulness. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations.
