Bergen New Bridge Medical Center is seeking a Nurse Practitioner for Off Site Services to provide comprehensive, compassionate, and evidence-based primary care services to individuals residing in County Correctional Facility and County Shelter. Under the direction of the BNBMC Occupational Health Medical Director and County Facility Leadership, the Nurse Practitioner will exercise sound clinical judgment and advocate for the health and safety of patients while balancing the operational requirements of the correctional and shelter environments. The role involves providing comprehensive care management, chronic disease management, and preventative care and wellness using the nursing process (assessment, planning, intervention, evaluation, and teaching). The Nurse Practitioner will complete assessments, order and interpret tests, prescribe medication, provide treatments, and manage follow-up care. Responsibilities also include completing employee physicals, maintaining medical records, promoting productivity, providing patient education, adhering to policies, participating in performance improvement initiatives, and fostering teamwork. The position requires interpreting x-rays and lab values, making case presentations to physicians, developing problem lists, documenting patient interactions, providing discharge instructions, and participating in quality improvement efforts. The role also emphasizes adherence to the Medical Center’s Code of Conduct, Mission, Vision, and Values, and maintaining excellent customer service standards.
