About The Position
Humana's Primary Care Organization is a fast-growing provider of senior-focused, value-based care, with 340+ centers across 15 states under the CenterWell and Conviva brands. We're looking for a part-time (VSP) Primary Care Nurse Practitioner who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric patients.
Requirements
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Current national certification (AANP or ANCC)
- Active, unrestricted NP license in SC.
- DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire)
Nice To Haves
- 2+ years Nurse Practitioner experience
- Experience in primary care, or Medicare Advantage settings with geriatric patients
- Experience with Athena EMR
- Familiarity with HEDIS quality indicators and Medicare guidelines.
Responsibilities
- Provide virtual primary care services for patients supporting Upstate South Carolina centers.
- Maintain accurate documentation and medical records
- Coordinate referrals and collaborate with specialists
- Partner with interdisciplinary teams for holistic care
