About The Position

The Nurse Practitioner provides comprehensive, patient-centered primary care to older adults and individuals residing in rehabilitation centers, skilled nursing facilities, assisted living communities, and independent living settings. The role focuses on assessing and managing acute and chronic health conditions, monitoring changes in patients’ health, developing individualized treatment plans, and supporting patients throughout recovery and long-term care. The Nurse Practitioner collaborates closely with physicians, nurses, caregivers, families, hospice, home health, rehabilitation, and other healthcare professionals to ensure coordinated and continuous care. Responsibilities may include performing physical assessments, ordering and interpreting diagnostic tests, managing medications, addressing acute illnesses and chronic disease, providing dementia and geriatric care, and helping patients and families understand treatment options and goals of care. By providing medical care directly within residential and long-term care settings, the Nurse Practitioner helps improve accessibility, continuity of care, comfort, and overall quality of life while reducing unnecessary hospital visits and supporting patients through changes in their health and functional needs.

Requirements

  • Graduate of an accredited Nurse Practitioner program
  • Current APRN licensure and board certification
  • Prescriptive authority in the state of Kansas
  • Experience in primary care, geriatrics, or chronic disease management
  • Strong assessment, communication, and care coordination skills
  • Ability to work collaboratively within an interdisciplinary team
  • CPR certification
  • Reliable transportation and ability to travel for home/community visits
  • Ability to work in clinic, home, and community-based settings

  • Experience working with older adult populations and complex medical conditions
  • Familiarity with electronic health records (EHR)
