Requirements

  • 2 years of related clinical nursing experience preferred.
  • Professional knowledge of nursing theory and practice, extensive knowledge of physical assessment, differential diagnosis, pathophysiology, and pharmacology.
  • Good oral and written communication skills.
  • Good interpersonal communication skills.
  • Speaks, reads, and writes English to the extent required by the position.
  • Graduation from an approved Nurse Practitioner program is required.
  • Current NJ licensure and certification as a Nurse Practitioner.
  • BLS, ACLS, and PALS certification required

Responsibilities

  • Serves as a clinical representative providing comprehensive, compassionate, and evidence-based primary care services to individuals residing in County Correctional Facility and County Shelter.
  • Under the direction of BNBMC Occupational Health Medical Director and County Facility Leadership, exercises sound clinical judgment and advocates for the health and safety of patients while balancing operational requirements of the correctional and shelter environments.
  • Renders medical care to patients in the ambulatory clinics and off-site practices.
  • Provides comprehensive care management, chronic disease management, preventative care and wellness utilizing the nursing process (assessment, planning, intervention, evaluation, and teaching).
  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient’s status.
  • Completes assessments including history and physical, medication reconciliation, problem list and develops a plan of care.
  • Performs assessment and plans for ongoing treatment for patients in the service.
  • Orders lab and diagnostic tests for patients as appropriate to formulate diagnosis, develop treatment plans.
  • Reviews results of lab and diagnostic tests and makes adjustments to the treatment plan accordingly.
  • Prescribes medication appropriate for the condition.
  • Provides treatment appropriate for the condition i.e.: wound care, respiratory treatments, basic emergency treatments.
  • Plans a response for follow-up for patients with abnormal tests results and intervenes with patient according to medical center policy and procedure.
  • Refers to specialist, collaborates with care team for follow-up care.
  • Completes employee physicals as assigned.
  • Assures all entries to the medical record, paper or electronic, are appropriate, timed, dated, signed and legible.
  • Promotes throughput for the service by achieving and maintaining medical center expectations for productivity.
  • Provides patient education and feedback to promote health and wellness.
  • Adheres to all medical center policies and procedures and bylaws of the medical staff.
  • Participates in department performance improvement initiatives, collects and aggregates data as needed, works with leadership to develop a plan of correction.
  • Fosters and promotes teamwork with other staff members of the service.
  • Communicates pertinent information to the supervisor of the service.
  • Performs comprehensive physical, psychosocial, and mental examinations, including developmental and functional status determination.
  • Interprets x-rays, with assistance from physicians, in order to formulate diagnosis.
  • Interprets lab values in order to correct abnormalities.
  • Evaluates consultations and makes concise case presentations to physicians, differentiating normal from abnormal findings and suggesting differential and presumptive diagnosis.
  • Develops a prioritized comprehensive problem list.
  • Documents in the patient’s record all pertinent patient interactions and care.
  • Provides discharge instructions with prescriptions given and follow-up visits arranged.
  • Participates in interdisciplinary quality improvement efforts to improve clinical practice.
  • Understands and adheres to the Medical Center’s Code of Conduct.
  • Familiar with the Medical Center’s Mission, Vision, and Values Statements.
  • Demonstrates customer service: respect, flexibility, knowledge, confidence, professionalism, and pleasant attitude, patience and helpfulness. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations.
  • Participates in continuing education activities; attends scheduled in-service programs; shares information gained at conferences with colleagues.
