The Nurse Practitioner works within the Harm Reduction framework to provide low-barrier, trauma-informed medical care to individuals experiencing homelessness and those disconnected from traditional healthcare systems. This role delivers mobile medical services across TNM’s outreach platforms, including Street Medicine, Health Outreach Bus, CTA outreach, shelter-based services, and youth programming, while bridging individuals to ongoing primary care, specialty care, and other community-based services. The purpose of this role is to deliver comprehensive, harm-reduction–oriented medical care in nontraditional settings; reduce barriers to care for unsheltered and marginalized populations; support engagement in substance use treatment, mental health care, and chronic disease management; strengthen referral pathways and partnerships; ensure compliance with regulatory and funding requirements; and contribute to ongoing program development and quality improvement.
