About The Position

Enhabit Home Health & Hospice is hiring a Nurse Practitioner to provide medical care to hospice patients. The role includes determining hospice eligibility, managing pain and symptoms, and participating in patient care as a member of the interdisciplinary team. The position requires an active nurse practitioner license in the state of practice, knowledge of applicable regulatory requirements, basic technology skills, CPR certification, and the ability to meet driving and transportation requirements.

Requirements

• Graduate of an approved school.
• Licensed as a nurse practitioner in the state where currently practicing.
• Demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Basic demonstrated technology skills, including operating a mobile device.
• Valid state driver license.
• Maintain automobile liability insurance as required by law.
• Maintain dependable transportation in good working condition.
• Ability to safely drive an automobile in all types of weather conditions.
• CPR certification.

Nice To Haves

• At least five years of nursing experience, including a minimum of one year in hospice.
• At least three years of clinical registered nurse practitioner experience.
