About The Position

Prairie Band Health Services is seeking an experienced Nurse Practitioner to provide services at Ute Health Center (UMUHC) in Towaoc, Colorado. This role offers competitive compensation and the opportunity to make a significant impact. The position involves providing a full range of medical services for outpatients of all ages, serving on the Patient Centered Medical Home team, making referral recommendations to the Clinical Director, and participating in treatment planning. Additional duties may be assigned by a supervisor.

Requirements

• Active, Unrestricted Medical License: Must hold a current and unrestricted license to practice as a Nurse Practitioner in any U.S. state, the District of Columbia, or the Commonwealth of Puerto Rico.
• Basic life Support (BLS) certification
• Advanced Cardiac Life Support (ACLS) certification
• Pediatric Advanced Life Support (PALS) certification
• Advanced Trauma Life Support (ATLS) certification
• Minimum 3 years of experience in diagnosis and treatment of a broad range of adult and pediatric conditions
• Must be a U.S. citizen living primarily within the United States.
• Must be willing and able to complete and pass the federal security clearance process.

Responsibilities

• Provide a full range of medical services including evaluation, diagnosis, consultation & referral, specialty referrals for outpatients of all ages.
• Serve on the Patient Centered Medical Home team.
• Make referral recommendations to the Clinical Director as required ensuring appropriate specialty care, follow-up, continuity of care and ensuring appropriate forms are completed.
• Participate in treatment planning with the medical department and for individual patients.
