About The Position

We’re hiring a Nurse Practitioner who wants predictable weekday hours, meaningful relationships with senior patients, and the opportunity to earn $143,000+ through base salary and incentives. You’ll step into an outpatient primary care role focused on seniors with complex chronic conditions. You’ll have your own care team and strong leadership support while practicing within a value-based care model.

Requirements

• Nurse Practitioner with 2+ years of outpatient primary care experience
• Comfortable caring for seniors with multiple complex conditions
• Hold an active, unrestricted license and current DEA registration
• Value patient-centered, team-based care

Nice To Haves

• Geriatric experience is a plus

Responsibilities

• Monday–Friday, 8:00 AM–5:00 PM
• See approximately 10–12 patients per day
• Manage a maximum panel of 650 patients
• Participate in call frequency based on the provider team size
• Work with a dedicated MA, scheduler, and referral specialist
• Practice within a value-based care model focused on quality and outcomes
• Provide care in a supportive, senior-focused clinical environment
• Perform suturing, joint injections, skin biopsies, and basic wound care
