About The Position

Heart’n Soul Hospice is seeking a Nurse Practitioner to support its mission of providing access to high-quality, compassionate care for people facing life-limiting illness and their families. The role involves patient and family education, palliative-care consultation, collaboration on plans of care, communication with physicians and the interdisciplinary group, and timely documentation. Requirements

• Graduate of a baccalaureate school of nursing.
• Current state license as a Registered Nurse and as an Advanced Practice Nurse (APN).
• Current driver’s license and reliable transportation.
• Provide ongoing valid and current automobile liability insurance.
• Two years of experience as an APN in a clinical care setting.
• Comprehensive knowledge of pharmacology, palliative care, and end-of-life care.

Nice To Haves

• Experience in hospice or home health is preferred.

Responsibilities

• Consult with and educate patients and families about disease processes, self-care techniques, and end-of-life care.
• Consult with and assist the interdisciplinary group (IDG) in developing patient and family plans of care to assure quality care.
• Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with the hospice physician.
• Inform physicians and other personnel of changes in patient needs and intervention outcomes.
• Evaluate patient and family responses to care.
• Submit all documentation in a timely manner.
• Perform other duties as required.
• Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
