About The Position

We are seeking an experienced Nurse Practitioner with a strong background in wound care and a solid understanding of vascular disease and lower-extremity circulation. This position will focus on the evaluation and management of patients with chronic, complex, and non-healing wounds, including wounds associated with diabetes, peripheral arterial disease, chronic venous insufficiency, pressure injury, edema, and other medical conditions that interfere with healing. Because vascular disease is a significant contributor to many chronic lower-extremity wounds, the ideal candidate must be comfortable identifying signs of arterial insufficiency, venous disease, impaired perfusion, ischemia, and other vascular barriers to wound healing.

Requirements

• Active Nurse Practitioner license in the applicable state.
• Graduate degree from an accredited Nurse Practitioner program.
• Current national Nurse Practitioner certification.
• Current BLS certification.
• Strong clinical assessment and documentation skills.
• Ability to independently manage a patient panel while collaborating with physicians and other providers.

• Previous wound-care experience strongly preferred.
• Vascular, podiatric, surgical, or lower-extremity clinical experience highly desirable.

Responsibilities

• Perform comprehensive wound-care evaluations and develop individualized treatment plans.
• Assess, measure, classify, photograph, and document wounds and healing progression.
• Evaluate causes of delayed healing, including arterial insufficiency, venous disease, diabetes, infection, pressure, edema, nutrition, and other comorbidities.
• Perform wound debridement and other wound-care procedures within the Nurse Practitioner's training, competency, scope of practice, and applicable regulations.
• Select appropriate wound-care therapies based on clinical presentation and treatment goals.
• Identify signs of arterial insufficiency, venous hypertension, ischemia, infection, and other complications.
• Evaluate lower-extremity vascular status and recognize when additional vascular testing or consultation is appropriate.
• Coordinate vascular studies and specialist referrals when clinically indicated.
• Collaborate with vascular specialists, podiatrists, primary care providers, home health agencies, and other members of the multidisciplinary care team.
• Participate in limb-preservation strategies designed to reduce complications and prevent avoidable amputations.
• Provide patient and caregiver education regarding wound care, pressure reduction, edema management, vascular risk factors, and prevention of recurrence.
• Monitor treatment outcomes and modify care plans based on patient response.
• Maintain thorough, accurate, and timely clinical documentation.
