Nurse Practitioner (Wound Care & Dermatology)
About The Position
A growing healthcare organization is seeking a compassionate and clinically confident Nurse Practitioner (NP) to join their Detroit, Michigan team. This role provides wound care and dermatologic services within a structured outpatient PACE clinic environment, serving senior populations. It offers a provider-focused opportunity with a consistent weekday schedule, established patient flow, strong clinical support, and the ability to make a meaningful impact caring for medically complex geriatric patients. Unlike traditional mobile or home-health roles, this position operates within coordinated clinic settings where providers function as an extension of the interdisciplinary care team. The ideal candidate enjoys autonomy, meaningful patient relationships, and collaborative senior-focused care without the administrative burden of private practice.
Requirements
- Active Nurse Practitioner license in Michigan
- Comfortable working independently within clinic-based environments
- Strong bedside manner and communication skills
- Reliable transportation for local travel between clinic sites
Nice To Haves
- Experience in wound care, geriatrics, dermatology, post-acute care, or senior-focused healthcare
- Wound care certification
Responsibilities
- Perform comprehensive wound and skin assessments
- Develop and manage individualized wound treatment plans
- Provide ongoing wound care management and follow-up care
- Perform procedures including: Debridement, Compression wrapping, Wound dressing applications, Negative Pressure Wound Therapy (Wound VAC)
- Perform dermatologic and skin integrity evaluations
- Identify and manage common skin conditions and chronic wound concerns
- Collaborate closely with physicians, nurses, and interdisciplinary care teams
- Educate caregivers and staff on wound prevention and treatment strategies
- Maintain accurate and timely EMR documentation
- Travel locally between clinic locations within the Detroit market as needed
