This role offers the opportunity to be part of an organization as it expands into Ohio and helps build a new model of care for medically complex patients. You'll be part of a collaborative, interdisciplinary care model that brings together physicians, Nurse Practitioners, Registered Nurses, Licensed Clinical Social Workers, therapists, and other healthcare professionals to proactively manage the needs of medically complex patients. Through telehealth, remote care management, and close coordination with patients, families, caregivers, health plans, and primary care providers, the team focuses on prevention, early intervention, and better outcomes. Our patient population may include both Medicaid and Dual Eligible members, including individuals with: Complex chronic medical conditions, Physical disabilities, Traumatic brain injuries, Intellectual and developmental disabilities, Functional limitations, Medically complex elderly populations. The role is primarily virtual, with the expectation that a collaborative office presence will eventually be established in one of Ohio's major metropolitan areas as the market grows.
