About The Position

Nirvana Healthcare Management Services, in partnership with Salerno Medical Associates, LLP, is seeking an experienced, bilingual Nurse Practitioner to join our Quality Program in an office-based role in West Orange, New Jersey. This is a non-traditional clinical position centered on telehealth: the NP will conduct patient encounters by televisit and telephone in support of the practice's quality, care-gap closure, and population health initiatives across our multi-site primary care network. The ideal candidate is a strong communicator in both English and Spanish, comfortable engaging patients remotely, and familiar with payer requirements and quality metrics. This role is well suited to an NP who enjoys proactive outreach, care coordination, and helping patients close gaps in care.

Requirements

  • Active, unrestricted New Jersey Nurse Practitioner license in good standing.
  • Experienced Nurse Practitioner with a strong clinical background.
  • Fully fluent in both English and Spanish (verbal and written) - this is a required qualification.
  • Strong computer literacy, including comfort with electronic health records, telehealth platforms, and standard office software.
  • Excellent verbal and written communication skills with the ability to build rapport with patients remotely.
  • Reliable, organized, and able to work on-site five days per week, with availability one Saturday per month.

Nice To Haves

  • Knowledge of quality metrics and quality-reporting frameworks (e.g., HEDIS, Stars, or value-based care measures).
  • Prior experience working with insurance companies, payer programs, or managed-care quality initiatives.
  • Experience conducting telehealth or telephonic patient encounters.
  • Background in primary care, care management, or population health.

Responsibilities

  • Conduct patient encounters via televisit and telephone in support of the organization's quality program and care-gap closure efforts.
  • Engage patients in both English and Spanish to assess needs, review care plans, and promote adherence to recommended preventive and chronic-care services.
  • Identify and help close gaps in care related to quality measures (e.g., preventive screenings, chronic disease management, and medication adherence).
  • Document encounters accurately and completely in the electronic health record, ensuring data supports quality reporting and payer requirements.
  • Collaborate with insurance companies and payer programs, supporting initiatives tied to quality metrics, HEDIS/Stars-type measures, and value-based care arrangements.
  • Coordinate with physicians, care teams, and office staff across locations to ensure timely follow-up and continuity of care.
  • Support the Quality Program's ongoing reporting, tracking, and performance-improvement activities.
