Nurse Practitioner, Psych PMHNP - AgeWise
About The Position
As a senior member of the clinical team, the Nurse Practitioner provides direct psychiatric nurse practitioner care and medical treatment to members served. This is an 8 hours per week, On-Site position. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. AgeWise is a community-based program serving older adults. AgeWise serves a maximum of 600 individuals, aged 59+. Agewise's Strengths-Based Case Management is a consumer-driven and highly collaborative program. Teams work with members to identify needs and build a treatment plan that is collaborative and comprehensive. The goal is to assist people living in the community and ensure members are linked to housing, medical care, psychiatric care and natural supports. Agewise's Institutional Case Management (ICM) serves members who are conserved and placed in a long-term care facility. The program provides members with support in obtaining and maintaining benefits, care coordination, discharge planning, and advocacy. Members are assessed to ensure that they are placed in the least restrictive environment possible.
Requirements
- Bachelor's Degree in Nursing Science and an advanced degree in a healthcare science as required by the State Licensing program
- Minimum of one (1) year of full-time experience, or its part-time equivalent, working in a program serving persons with mental disabilities
- Completion of an accredited Nurse Practitioner program
- American Nurses Credentialing Center (ANCC) Board Certified as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Licensure as an Advanced Practice Nurse Practitioner in the State of practice
- Active DEA and Furnishing licenses
