Position Summary: The Nurse Practitioner Post-Acute & Long-Term Care provides comprehensive primary and transitional medical care to residents in skilled nursing facilities, long-term care facilities, and assisted living communities. Working collaboratively with physicians, facility staff, and interdisciplinary healthcare teams, the APRN assesses, diagnoses, treats, and monitors acute and chronic health conditions while promoting quality outcomes and continuity of care. The APRN develops, implements, and evaluates individualized treatment plans, provides patient and family education, and serves as a clinical resource to facility staff and community providers. This role demonstrates excellence in advanced nursing practice while supporting the mission, vision, and values of Bluegrass Care Navigators.

Requirements

  • Graduate of an accredited Nurse Practitioner program with a Master's degree in Nursing required.
  • Current Advanced Practice Registered Nurse (APRN) licensure through the Kentucky Board of Nursing required.
  • Current certification as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) required.
  • Minimum of three (3) years of general nursing experience required.
  • Current CPR/BLS certification required.
  • Excellent written and verbal communication skills, including the ability to complete accurate and timely clinical documentation.
  • Demonstrated ability to work independently with minimal supervision while maintaining effective collaboration within an interdisciplinary team.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.

Nice To Haves

  • Clinical experience post-acute care, long-term care, skilled nursing facilities, assisted living, geriatrics, hospice, palliative care, or primary care preferred.
  • Demonstrated advanced clinical assessment, diagnostic, and treatment skills with the ability to manage complex patient populations.
  • Strong knowledge of evidence-based practice and the ability to apply current clinical knowledge to patient care.
  • Excellent clinical judgment, critical thinking, and problem-solving abilities.
  • Strong interpersonal, communication, collaboration, and conflict resolution skills.
  • Ability to effectively prioritize and manage multiple responsibilities in a fast-paced healthcare environment.
  • Strong organizational and time management skills with the ability to coordinate resources effectively.
  • Ability to adapt to change and support process improvement initiatives.
