Long Term and Post-Acute Care APP
About The Position
MedCap Health is seeking a full-time, part-time, flexible schedule Long Term and Post-Acute Care APP to help reshape the future of post-acute care. This role offers flexibility, autonomy, and a true sense of purpose. As a key member of a multidisciplinary team, you will play a central role in patient care, managing patients with acute and chronic diseases. You will collaborate directly with the attending physician to provide the highest quality of care and effectively manage patients with debilitating chronic diseases, improving their quality of life. You will also communicate and work seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care.
Requirements
- A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
- 1+ years of experience in post-acute/SNF/LTC
- Prior post-acute, long-term, and/or skilled nursing facility experience
Responsibilities
- Managing patients with acute and chronic diseases
- Collaborating directly with the attending physician to provide the highest quality of care
- Effectively managing patients with debilitating chronic diseases, improving their quality of life
- Communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care
