About The Position

Community Health Network is seeking a Psychiatric-Mental Health Nurse Practitioner (PMHNP) to join our behavioral health team providing integrated care in the primary care setting. This provider will practice within an outpatient primary care clinic located on the South side of Indianapolis, working in collaboration with a team that includes an LCSW and primary care physicians and advanced practice providers. Ideal candidates will be highly accountable, independent, and have a desire to deliver direct and collaborative patient care.

Requirements

• Psychiatric-Mental Health Nurse Practitioner (PMHNP) qualification.
• Licensure and certification consistent with requirements within the state of Indiana.
