About The Position

We are seeking a dedicated and highly skilled Nurse Practitioner (NP) to join our growing vascular team. The successful candidate will practice autonomously within their legislated scope, independently assessing patients in consultation, ordering and interpreting appropriate diagnostic imaging, and developing and implementing treatment plans for vascular conditions. Working in close collaboration with our Vascular Surgeons and interdisciplinary clinical teams, the NP will play a key role in delivering high-quality, patient-centred vascular care across the continuum — from initial consultation through treatment planning and follow-up. This role is ideal for a Nurse Practitioner who thrives in a dynamic, multidisciplinary environment and is passionate about advancing access to timely, evidence-based vascular care.

Requirements

  • Nurse Practitioner in good standing with the College of Nurses of Ontario (CNO).
  • Completion of a recognized Nurse Practitioner program.
  • Minimum 2 years of clinical experience as a Nurse Practitioner.
  • Demonstrated ability to practice autonomously with strong clinical judgment and decision-making skills.
  • Excellent interpersonal, communication, and patient education skills.
  • Proficiency with EMR systems and clinical documentation.
  • Valid Ontario driver’s license and reliable transportation required.

Nice To Haves

  • Experience in vascular surgery, cardiovascular care, internal medicine, or related specialties strongly preferred.

Responsibilities

  • Independently conduct comprehensive patient consultations, including detailed medical histories, physical examinations, and vascular assessments.
  • Order, review, and interpret appropriate diagnostic imaging and laboratory investigations in accordance with NP scope of practice.
  • Formulate differential diagnoses and develop individualized treatment and management plans for vascular patients, including conservative, interventional, and surgical pathways.
  • Work collaboratively with Vascular Surgeons to coordinate care, escalate complex cases, and support surgical decision-making when indicated.
  • Provide ongoing follow-up care, including post-procedural assessments, wound care, symptom monitoring, and longitudinal management of vascular conditions.
  • Educate patients and families on vascular disease processes, treatment options, procedural expectations, and long-term risk reduction strategies.
  • Triage and manage patient concerns, ensuring timely intervention, continuity of care, and appropriate referrals as required.
  • Accurately document all assessments, orders, clinical decisions, and patient encounters within the EMR in accordance with professional, legal, and organizational standards.
  • Support clinical operations by contributing to care coordination, workflow efficiency, and interdisciplinary communication.
  • Travel regularly to Oakville, Burlington, and Stoney Creek clinics, with flexibility to attend other regional clinic locations as required.
  • Participate in quality improvement initiatives, policy development, and ongoing professional development to maintain best practices in vascular care.
