About The Position

Senior Health Physician Services is a growing medical practice dedicated to providing exceptional care to residents of skilled nursing facilities, assisted living communities, and home-based patients throughout the Mountain West. Our providers serve as attending providers, primary care providers, and medical directors, delivering high-quality, patient-centered care to medically complex senior populations. We are committed to supporting our providers with efficient systems, responsive leadership, and a collaborative care model that allows clinicians to focus on what matters most—excellent patient care. Senior Health is seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) licensed in Wyoming to provide care in a Skilled Nursing Facility (SNF) on a part-time basis (approximately 16 hours per week). This position offers the opportunity to develop meaningful relationships with residents, collaborate closely with nursing staff, and practice medicine in an environment that values clinical judgment, autonomy, and quality outcomes.

Requirements

  • Active Wyoming Nurse Practitioner or Physician Assistant license
  • Current DEA registration (or ability to obtain)
  • Ability to travel to assigned skilled nursing facilities
  • Strong clinical assessment and decision-making skills
  • Excellent communication and documentation abilities
  • Ability to work independently while collaborating with interdisciplinary teams

Nice To Haves

  • Skilled Nursing Facility (SNF) experience
  • Geriatric medicine experience
  • Internal medicine, family medicine, hospital medicine, or post-acute care experience
  • Knowledge of Medicare, long-term care, and value-based care practices

Responsibilities

  • Conduct routine, follow-up, and medically necessary visits for SNF residents
  • Manage acute and chronic medical conditions
  • Evaluate new admissions and post-hospital transitions
  • Collaborate with facility nursing staff, therapy teams, specialists, and families
  • Develop and implement treatment plans
  • Document patient encounters accurately and timely within the EHR
  • Participate in quality improvement and care coordination initiatives
  • Provide proactive communication to facility leadership and interdisciplinary teams
