About The Position

Care New England is seeking a Nurse Practitioner or Physician Assistant to join its Integra at Home program. The program provides 24/7 clinical oversight for a select group of high-risk patients through nurse care management, in-home NP/PA visits, community paramedicine, and remote patient monitoring, with the goal of preventing unnecessary emergency department visits and hospital admissions. This position uses a hybrid care model combining in-person visits and telehealth. The average patient age is 60 or older. The role is full-time, with either a five-day or four-day work week and rotational paid call. Compensation includes a competitive salary and benefits, including health coverage, malpractice coverage, and an employer retirement match.
