About The Position

Open Door Community Health Centers is seeking a Primary Care Nurse Practitioner/Physician Assistant to join their integrated care team at the Fortuna Community Health Center in Fortuna, California. This role involves assessing and evaluating patient healthcare needs, performing diagnostic and therapeutic procedures, and managing patient care. Open Door Community Health Centers is committed to providing compassionate, whole-person care across Medical, Dental, Behavioral Health, and Pharmacy services on California's North Coast.

Requirements

  • Diploma from an accredited program with credentialing through the American Nurses Association or Physician Assistant Association.
  • Current, unrestricted California license.
  • Current DEA registration.
  • Current Board Certification in specialty of practice (or board eligible if a newer graduate).
  • Current CPR/BLS certification.
  • Valid California driver's license with an insurable driving record.

Responsibilities

  • Assesses and evaluates the health care needs of patients.
  • Performs diagnostic and therapeutic procedures.
  • Institutes and manages care for patients.
  • Performs related work as assigned.
