About The Position
Millennium Physician Group, part of Mosaic Health, is seeking an experienced and compassionate Nurse Practitioner (APRN) or Physician Assistant (PA-C) to join its growing primary care team in Jacksonville, Florida. The provider will manage an established and growing patient population in a collaborative, value-based care environment, with dedicated clinical and administrative support. This full-time, outpatient primary care role focuses on preventive care, chronic disease management, quality measures and HCCs, and building long-term patient relationships. Providers are supported by physicians and APPs, care managers, clinical pharmacists, social workers, hospitalists, transitional care teams, home health services, and 24/7 nurse triage support. The practice uses an AI-integrated Athena EMR and offers resources intended to help providers focus on patient care and practice at the top of their license. The position offers opportunities for professional growth within one of the Southeast’s largest independent physician groups. Jacksonville provides coastal living, outdoor recreation, diverse communities, dining, entertainment, professional sports, and no Florida state income tax.
Requirements
- Current Florida APRN or Physician Assistant license.
- BLS and ACLS certifications.
- Commitment to patient-centered, value-based care.
- Strong communication and relationship-building skills.
- Collaborative, team-oriented approach.
