About The Position

Care New England is seeking a Nurse Practitioner or Physician Assistant for their Integra at Home program. This program provides 24/7 clinical oversight to high-risk patients through various methods including nurse care management, in-home NP/PA visits, community paramedicine, and remote patient monitoring. The primary goal is to prevent unnecessary emergency department visits and hospital admissions. This role involves a hybrid care model, combining in-person visits with telehealth. The average patient age is 60+.

Requirements

Graduate of an accredited Physician Assistant or Nurse Practitioner program
Certified by NCCPA or ANCC or AANPCB
Active Rhode Island PA, CSR, and DEA license or eligible.

Responsibilities

Provide evaluation and treatment of acute and acute-on-chronic medical conditions in the home or via MIH (Mobile Integrated Health) supported virtual visits.
Manage common acute diagnoses including COPD and CHF exacerbations, skin and soft tissue infections, pneumonia, viral upper respiratory infections, and urinary tract infections.
Deliver intensive in-home diagnostic and treatment pathways, including laboratory testing, imaging, respiratory therapies, and IV medications.
Recognize clinical deterioration and escalate care to the emergency department when patients cannot be safely managed in the home.
Work closely with a multidisciplinary team including case managers, nurses, physicians, social workers, and pharmacy.
Provide early post-discharge and high-risk follow-up visits for patients recently discharged from the hospital or skilled nursing facility to reduce readmissions.
Provide focused patient and caregiver education related to acute and chronic conditions.
Participate in interdisciplinary case reviews and quality improvement initiatives focused on acute care outcomes and hospitalization avoidance.
