About The Position

West Coast Wound & Skin Care is a mobile wound care company providing services to patients in their homes. The organization focuses on whole-patient health, communication with partner agencies, responsiveness to patient needs, and training and continuing education for its medical team. The Nurse Practitioner or Physician Assistant provides advanced wound care to patients in their homes or skilled nursing facilities (SNFs), traveling locally throughout the South Chicago, Illinois territory. Candidates must live in South Chicago, hold an Illinois NP or PA license, and be willing and able to drive to patient homes. This is not a remote position. No prior experience is required; extensive training and 24/7 support are provided. The role is offered as full-time, part-time, or 1099, with competitive compensation and malpractice insurance.

Requirements

Current Illinois state license as a Nurse Practitioner or Physician Assistant.
CPR certification.
Effective communication skills.
Excellent interpersonal and customer service skills.
Valid driver's license and at least the state-required liability auto insurance.
Must live in South Chicago, Illinois, and be willing and able to travel locally to patients' homes.
No prior experience is required.

Responsibilities

Select, order, and perform tests and procedures to best treat patients.
Perform wound care procedures as appropriate for the plan of care.
Define and document patient diagnoses.
Ensure patients are satisfied with their care.
Provide patient education and management.
Safely debride wounds using various methods during patient visits.
Apply topical wound care and other wound management products.
Record findings and treatment courses in patient charts, including follow-up notes.
