About The Position

Mass General Brigham Medical Group Northern Massachusetts, Inc. is seeking a Part-Time, 24-hour, Remote Nurse Practitioner or Physician Assistant to join their Virtual Clinical Support team. This role is designed for high-demand, high-volume patient support. The schedule for the 24-hour role is Tuesdays, Thursdays, and Fridays from 8:30 AM to 5:30 PM. The ideal candidate will possess strong primary care experience and a proven ability to manage a wide array of patient needs in a virtual environment. Responsibilities include conducting virtual patient consultations, processing medication refill requests, and coordinating patient care. This position is crucial for delivering timely, high-quality patient care, improving access to services, ensuring care continuity, and boosting the overall efficiency of healthcare delivery. By alleviating administrative and clinical workload for primary care providers, this role contributes to better patient outcomes and a smoother care experience.

Requirements

  • Master's Degree in Nursing required.
  • 2 years of licensed nurse practitioner experience required.
  • 5+ years of Nurse Practitioner or Physician Assistant experience in Primary Care required.
  • Prior experience with Epic required.
  • MA, NH, and ME NP or PA licensure required upon hire.
  • MA and NH DEA registration required upon hire.

Responsibilities

  • Handle a high volume of routine prescription refill requests, review patient history, and ensure refills are appropriate.
  • Respond to patient inquiries and concerns through the EHR that do not require direct PCP involvement.
  • Review incoming lab results, flag abnormalities, and initiate necessary follow-up actions.
  • Perform virtual consultations for non-urgent issues, providing preliminary assessments and management plans, escalating to PCP when needed.
  • Manage chronic conditions through virtual visits, ensuring patient adherence to care plans and making adjustments as necessary.
  • Manage referrals to specialists or other healthcare services, ensuring timely direction to the appropriate level of care.
  • Act as a liaison between the patient, PCP, and other specialists to ensure smooth communication and adherence to care plans.
  • Educate patients on their conditions, treatment plans, and self-care strategies during virtual interactions.
  • Participate in continuous process improvement, providing feedback and willingness to adopt new methods.
  • Participate in quality improvement initiatives and ensure compliance with clinical guidelines and organizational policies.
  • Stay updated on the latest clinical practices, telehealth trends, and EPIC EHR functionalities.
