About The Position
Advanced Practice Provider-Outpatient Transplant (Full-time) Piedmont Healthcare is seeking a Nurse Practitioner or Physician Assistant to join our Transplant Outpatient Team. This role is responsible for the ongoing management and coordination of care for liver transplant patients in a collaborative, multidisciplinary setting. The ideal candidate is clinically strong, highly organized, and comfortable practicing with a high degree of autonomy while managing complex transplant patients.
Requirements
- PA: Graduate of a Physician Assistants program with current license for the Georgia State Composite Board OR NP: Graduate from a school of nursing with current Georgia license for Advanced Practice Nursing and a Master’s degree in Nursing

Responsibilities
- Provide comprehensive evaluation and management of liver transplant recipients.
- Develop and implement individualized treatment plans based on clinical assessment, laboratory, and diagnostic findings.
- Monitor graft function, immunosuppressive therapies, and post-transplant complications.
- Educate patients and families regarding transplant care, medications, and lifestyle modifications.
- Coordinate care with transplant surgeons, hepatologists, coordinators, pharmacists, and other members of the multidisciplinary team.
- Perform minor procedures within scope of practice.
- Obtain informed consents as appropriate.
- Participate in transplant program quality initiatives, research activities, and multidisciplinary conferences.
- Serve as a clinical resource for staff and support educational initiatives.
- Participate in after-hours call coverage approximately once per month, including holidays.
