About The Position

We are a well-established, patient-focused family medicine practice dedicated to providing comprehensive, compassionate care to the Phoenix community. Our team values collaboration, quality improvement, and continuity of care. Providers enjoy a predictable weekday schedule, supportive clinical staff, and the opportunity to build meaningful, long-term patient relationships in an outpatient setting. The Nurse Practitioner or Physician Assistant will provide comprehensive primary care services in an outpatient family medicine setting. This role includes delivering preventive, acute, and chronic care to a diverse patient population while supporting quality initiatives and coordinated, team-based care. The provider will maintain accurate documentation and contribute to ongoing practice-wide quality improvement efforts.

Requirements

• Board Eligible or Board Certified Nurse Practitioner or Physician Assistant
• Valid Arizona medical license (or ability to obtain)
• Current DEA registration
• Strong patient communication and interpersonal skills

Nice To Haves

• Experience in outpatient primary care preferred
• Cover Letter and references are preferred but optional.

Responsibilities

• Perform annual wellness exams and provide preventive, acute, and chronic care
• Manage a diverse panel of patients across all ages
• Maintain accurate and timely documentation in the EMR
• Collaborate with clinical staff to ensure coordinated, patient-centered care
• Support managed care initiatives and quality programs
• Provide patient education to promote healthy lifestyle and disease prevention
• Participate in practice-wide quality improvement efforts
