Total Extended Care Services is a growing Physical Medicine and Rehabilitation (PM&R) group focused on helping patients regain function, improve quality of life, and reduce avoidable hospital readmissions in skilled nursing and sub-acute rehabilitation settings. Our clinicians work closely with interdisciplinary care teams to deliver consistent, patient-centered rehabilitation care in senior living communities. This role offers the opportunity to work alongside Board-Certified PM&R physicians in a specialized rehabilitation environment where your clinical judgment and collaboration directly impact patient recovery outcomes. Rather than working in a high-volume primary care setting, you will focus on helping patients improve mobility, functionality, pain management, and overall rehabilitation progress. This is an excellent career move for a Nurse Practitioner or Physician Assistant who values autonomy, interdisciplinary teamwork, flexible scheduling, and meaningful long-term patient impact.
